- comments: no hand washing signage for the prep hand washing sink in the small basement prep room. Must provide.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: box freezers throughout basement must not be stored under waste water piping. 4-204. 12-- missing lid for the bar ice bin. Must provide a lid. 4-501. 11-- torn rubber door gasket on the kitchen two door cooler. Must replace.

55. Physical facilities installed, maintained & clean
- comments: - all unused equipment, outdoor grill, wood pallets, cut tree logs and debris piled on the rear patio/garbage corral must be removed from the premises to prevent rodent harboring. 6-201. 11--- painted brick walls throughout several areas of the basement food prep areas and dry food storage with peeling, damaged paint/brick. Must repair all to be smooth and cleanable. -----must provide a smooth cleanable ceiling above the basement ice machine.

57. All food employees have food handler training
- comments: no proof of training or food handler certificates for any employees. Must provide.

58. Allergen training as required
- comments: no proof of allergen training or certificates for any city of chicago certified food mangers. Must provide.

64. Public health nuisance
- comments: observed the owner and an employee at the host stand inside the establishment not wearing a face covering. Must provide face coverings. Citation issued. 7-28-060.
