1. A hotline advocate who had been in the office within 48 hours of onset of symptoms alerted the Coalition to their positive COVID-19 diagnosis on Saturday, Nov. 28th. As of the filing of this complaint (11/30/20) those who were in contact with this hotline advocate by … See More
way of being on shift together have not been notified by the Coalition. It is undetermined whether this information was immediately shared with the Ingham County Health Department as directed by MIOSHA. In addition to this violation, another employee was diagnosed with COVID-19 and entered the workplace on Nov. 15/16. While the Coalition alerted the three employees who had come into contact with that person, the employer knowingly allowed one of those employees to return to the work site within 7 days of contact with the known case of COVID-19 without them quarantining for 14 days per the CDC recommendations. (Employees affected: 5 to 10) 2. As previously mentioned in MIOSHA Inspection #C1660359, the Coalition continues to force staff to physically report to the workplace and cite the cost and refusal of funders to cover that cost. This is despite many programmatic side employees and hotline supervisors serving as on-call support who take hotline call/texts/chats from a remote location. As mentioned above, many employees not employed as hotline advocates perform all responsibilities of the hotline from a remote location as on-call support while hotline staff are required to report to work. (Employees affected: Up to 12) 3. Neither increased frequency of cleaning nor deep cleaning was performed after both aforementioned instances of the employer being aware that an employee had entered the workplace with a known case of COVID-19. Additionally, the cleaning company employed by the Coalition (AP Cleaners) do not wipe down high-touch surfaces with EPA Approved Disinfectant in their cleaning protocols and merely vacuum and empty the garbage cans. Additional office spaces have been opened up for social distancing purposes, but those offices are not cleaned in a professional capacity other than vacuuming. (Employees affected: Up to 12) 4. No one who is on site at all times that workers are present at the Coalition have been designated or received additional training to be considered COVID-19 safety coordinators. This complaint was also present in MIOSHA Workplace Inspection #C1660359 and was not addressed in the Coalitions Preparedness and Response Plan and continues to be ignored. The Coalition has assigned shift leads to check that everyone who enters the building takes the MiSymptoms app questionnaire but has not told them they are designated COVID-19 safety coordinators. Additionally, there is not always a shift-lead present on all shifts when workers are present.

1. A COVID preparedness and response plan has not been developed or provided to employees. 2. No one has been designated as a worksite supervisor and there is not a person on site at all times with any supervisory capacity who may claim that is their role. … See More
Only hotline advocates are required to report to work in-office. 3. Employees have not been provided COVID-19 training regarding workplace infection-control practices, proper use of personal protective equipment, steps the employee must take to notify the business or operation of any symptoms of COVID-19 or a suspected or confirmed diagnosis of COVID-19, or how to report unsafe working conditions. 4. Signs that encourage staying home when sick, cough and sneeze etiquette and proper hand hygiene practices have not been posted in the workplace. 5. A daily entry self-screening protocol for all employees or contractors entering the workplace, including at a minimum a questionnaire covering symptoms and suspected or confirmed exposure to people with possible COVID-19, has not been implemented. Employees have merely been instructed to take their own temperature and log that temperature. 6. Facility cleaning and disinfection to limit exposure to COVID-19, especially on high-touch surfaces (e.g., door handles), paying special attention to parts, products and shared equipment (e.g., tools, machinery, vehicles) has not occurred. Staff have merely been instructed to wipe down their own work stations with no increased professional cleaning occurring in shared spaces. 7. Protocols are not in place to clean and disinfect the facility in the event of a positive COVID-19 case. 8. A response plan for dealing with a confirmed infection in the workplace, including protocols for sending employees home and for temporary closures of all or part of the workplace to allow for deep cleaning has not been established. 9. Promoting remote work to the fullest extent possible has not been accomplished; MCEDSV hotline supervisory staff and programmatic staff have been working from home since the Stay Home, Stay Safe order was issued, whereas hotline advocates have been required to report to the physical office.

response from service@ risktic. com the first time when I emailed them a day after the package was said to have been delivered and I was instructed to wait 48 hours. I waited then sent another email (the only source of communication) to the same address but this time no response. I sent another email asking for a complete refund, the prior email I only requested the dresses. The response from this email was a different address support@ lixvb. com and asked if they could refund 50%. This is a complete scam. Don’t buy from them. It’s been 30 day and still no dresses.
