1. The employer is not following CDC guidelines regarding informing employees of workers who have tested positive for COVID-19. Three employees tested positive and the coworkers were not informed.Applicable OSHA standard(s): Section 5(a)(1) General Duty Clause of the Occupational Safety and Health Act of 1970.
2. The employer is not following CDC guidelines in regards to the use of facemasks or face coverings. Managers do not wear masks throughout the day or wear the mask improperly, exposing their nose and mouth.

3. The employer is not following CDC guidelines in regards to the use of facemasks or face coverings. The employer is not providing appropriate masks and in adequate number for employees.

4. Employees are required to perform work duties that require employees to be within 6 feet of each other and the customers. The employer is not following CDC guidelines regarding maintaining 6 feet of social distancing.

5. There is insufficient quantity of hand sanitizer at the site for employees to use during the frequent use of high contact surfaces such cash registers and door handles. Hand sanitizers frequently run out and are not refilled.

6. The employer is not following CDC guidelines for increased cleaning and sanitizing of the establishment, after employees have tested positive for COVID-19.
