1. The employer has issued N95 masks to employees, however the employees have not been fit tested and have masks that do not fit properly. 2. There have been COVID-19 cases, however the employer is not performing proper cleaning inside rooms of the clinic. The rooms are
not cleaned prior to new patients being evaluated.
