1. Employees are exposed to health hazards from COVID-19, because they are required to wear N95 respirators while caring for COVID-19-positive residents, and they have not been fit-tested. 2. Employees are exposed to health hazards from COVID-19, because employees are wearing N95 respirators provided with exhalation valves.

Employees are exposed to health hazards from COVID-19, because employees wearing N95 respirators while caring for COVID-19-positive residents are required to wear them out to their cars, and store them in their cars. 4. Employees are exposed to health hazards from COVID-19, because employees wearing face shields while caring for COVID-19-positive residents, are required to wear them out to their cars, and store them in their cars. 5. Employees are exposed to health hazards from COVID-19, because employees caring for COVID-19-positive residents are required to re-use, for up to a week, personal protective equipment such as, but not limited to, face shields and N95 respirators, which have not been disinfected. 6. Employees are exposed to health hazards from COVID-19, because they are caring for COVID-19-positive residents, and the employer has not provided head or foot coverings. 7. Employees are exposed to health hazards from COVID-19, because employees are required to wear face coverings other than N95 respirators in the facility, which has COVID-19-positive residents.
