1. The employer did not ensure that facial coverings were provided and worn by all unvaccinated employees in building 687. T8 CCR 3205(c)(6)(A). 2. The employer did not ensure to review applicable orders and guidance from the State of California and local health departments related to COVID-19
hazards and prevention, specifically the local requirements for employee facial covering requirements. T8 CCR 3205(c)(2)(F). 3. The employer did not ensure that employees who are exposed to eye hazards such as punctures, abrasions, contusions or burns are protected by means of proper eye protection. T8 CCR 3382(a). 4. The employer did not ensure that the business is maintained, insofar as is practicable, to prevent the entrance or harborage of rodents and other pests in the establishment. T8 CCR 3362(f).
