but not limited to COVID-19. 2. Employees are required to carry heavy pieces of granite across the floor, which has exposed holes that are up to 2 feet deep and wide enough for a foot to get in. This condition exposes employees to trip and fall hazards. 3. The walking and working surfaces in the shop are not kept in a clean and orderly manner due to hoses and random trash found all over the floor. 4. When rainwater collects on the shop floor, extension cords are submerged in the standing water, exposing employees to electrical hazard. 5. The 2 overhead cranes and the forklift have never been inspected. These equipment are used to lift and move granite slabs weighing approximately 1000lbs throughout the shop. 6. Employees operating the forklift and the 2 overhead cranes are not trained to operate the equipment. 7. In the front left section of the shop, the storage racks and/or stands for the full granite slabs that weigh about 700-1200lbs are rusted and unstable. Approximately 20 granite slabs are placed of the rusty and unstable racks/stands.

powder discoloration in some packets leading to decreased potency. The issue was discovered during an accelerated stability test in addition to customer complaints. Sapropterin Dihydrochloride Powder for Oral Solution 100 mg was distributed nationwide to wholesalers/retailers. Dr. Reddy’s Laboratories Inc. has not received any reports of adverse events related to this recall to date.

Reduced efficacy of the product would result in elevated Phenylalaninemia (Phe) levels in patients. Chronically elevated Phe levels in infants and children are likely to cause permanent neurocognitive deficits, including permanent and irreversible intellectual disability, developmental delay, and seizures. Furthermore, elevated Phe levels during pregnancy, especially in early gestation, are associated with microcephaly and congenital heart disease.

The product is indicated to reduce blood phenylalanine (Phe) levels in adult and pediatric patients one month of age and older with hyperphenylalaninemia (HPA) due to tetrahydrobiopterin-(BH4-) responsive Phenylketonuria (PKU) and is packaged in individual packets, 30 per carton. The affected Sapropterin Dihydrochloride Powder for Oral Solution 100mg lots include the following:

Dr. Reddy’s Laboratories Inc. is notifying its distributors and customers by recall notification letters and is arranging for returns of all recalled products. Consumers who have Sapropterin Dihydrochloride Powder for Oral Solution 100 mg which is being recalled should contact their physician before stopping use of the product.
