Welding is occurring on carbon steel and proper respirators are not being worn. Harness and lanyards are not being inspected. Fire hazards exist including the improper storage of compressed gas cylinders (acetylene and oxygen), smoking near flammable storage areas and compressed gases and fire extinguishers are not located within 75 feet of work locations. Multiple electrical hazards exist including , no assured grounding program in place, open panels and exposed busbars, extension cords being used in lieu of permanent wiring, damaged and improperly repaired cords are being used. Tool and equipment inspections are not being conducted. Damaged power cords are being used. Damaged welding leads are being used. Back alarms are not operational. Permit required confined spaces are being entered without checking atmosphere. There is no confined space training for employees and no competent person onsite. Safety Data sheets are missing Handwashing facilities or alternatives are not available. Crane lifts are being conducted without ground bearing pressure calculations. Vaults and other voids exist in the area. CoVID protocols are not being followed. Employees are not following emergency action plans for weather. There is no bulletin board with the required postings.

High levels of vitamin D fed to a pet over a short period (weeks/months) should not cause undue concern. Over a longer period of feeding, ingestion of elevated levels can lead to potential dog health issues. Vitamin D, when consumed at very high levels, can lead to serious health issues in dogs including renal dysfunction.

See More2020/05/10 L/N: 20200510-3 of imc Wash-Free Hand Sanitizer 50 ml, 100 ml, 300 ml and Wash Free Hand Sanitizer 300 ml lot 2020/05/11 and Thrifty White Pharmacy Wash-Free Hand Sanitizer 300 ml lot 2020/05/11 listed in the table below to the consumer level. The products are being recalled due to the potential presence of methanol (wood alcohol).

Risk Statement: Substantial methanol exposure can result in nausea, vomiting, headache, blurred vision, permanent blindness, seizures, coma, permanent damage to the nervous system or death. Although all persons using these products on their hands are at risk, young children who accidentally ingest these products and adolescents and adults who drink these products as an alcohol (ethanol) substitute, are most at risk for methanol poisoning. To date, Shane Erickson, Inc. has not received any reports of adverse events related to the product(s) of this recall. Recalled Products:

- IMC Wash-Free Hand Sanitizer | Wash-Free Hand Sanitizer | Thrifty White Pharmacy Wash-Free Hand Sanitizer 300 ml, 10.14 oz (8 19845 00599 2). Lot: 43962. Expiration date: 45056

The product is used as a hand sanitizer for handwashing to decrease bacteria on the skin when soap and water are not available. All products are packaged in a plastic bottle. The 50 ml are in a HDPE opaque white bottle with a blue cap, the 100 ml is in a clear PET bottle with a clear cap and the 300 ml are in a clear PET bottle with a black pump. The product can be identified by checking the product size, UPC details, lot number and expiration date on the bottle containing the product. The product can also be identified by this information on the back label: Made in China, Distributed by IMC 4284 Shoreline Dr, Spring Park, MN 55384, YUEWEIXIAOZHENGZI (2020) - 04 - No. 0023.

Some of the product has custom front labels (see examples below) the back label on the custom labels have the same identification information as previously mentioned above. The hand sanitizer was distributed Nationwide in the USA to wholesale and retail customers.

Shane Erickson, Inc. is notifying its distributors and customers by mail/email and is arranging for the return of all recalled products. Consumer, distributors, and retailers that have product, which is being recalled, should stop use or distribution and return to place of purchase. I grabbed her up and rushed her to the vet. She had been poisoned by the dream bones her favourite treat. S... See Morehe started hemorraging blood out her butt, after transfusion it stopped around 4.30 am sept 27th then they called me to say her heart stopped and they revived her and she was on a ventolator and when I got there awww so sad poor thing. She was now bleeding in her lungs OH MY GOD. She passed away that morning. I cried for 2 and a half hours in the room with her dead I just could not bear to leave her. The vet bill was over 2400 dollars and she was gone in 12 hours after eating that stupid dream bone. I still have the package of treats. I bought them at Walmart. She was not even 2 years old yet. “My puppy Angus, at age 5 months, ate a small amount of a dream bone on Jan 9 in the afternoon, and then started crying and pacing, not eating any more. About 5 pm, he refused to eat, and went to lie down in his bed, not begging at our table while we ate. He seemed stiff, and lay down with his neck... See Morestretched out. Soon he was unresponsive and glassy-eyed, and stiff as a board. I took him to the Emergency vet, who did not think that there was any obstruction, and while at the vet's he improved, so I brought him home for observation. The vet said that it could be the onset of a neurological condition, or something in the Dreambone. He recovered, is now 18 months (it is more than a year later) and has never had a similar episode. By a process of elimination, it is probable that the Dreambone caused this reaction, and that it wasn't lethal because he only ate a small amount. See Less
