1. Employees are exposed to COVID-19 hazards, in that, employees are not wearing facial covering while working at the Indiana belt by post F3 (MPA), the 185 belt by post F1 (MPA), and the USS machine by posts Y&Z (P2). 2. The employer has not posted the 2020 OSHA 300a summary in a conspicuous place or places where notices to employees are customarily posted. 3. Clerks, mail handlers, and management are permitted to operate electric powered jacks before successfully completing required training. 4. Employees are not following procedures and are using electric powered jacks in lieu of tugs to pull containers (APCs and casters) (MPA, P1, and P2). 5. Employees using the rental Toyota forklifts are defeating the seatbelt interlock and NOT properly wearing the seatbelt by buckling the seatbelt and sitting on the seatbelt or wrapping the seatbelt around the front or bottom of the seat and buckling it (MPA, P1 and P2). 6. Employees are wearing earbuds or using wireless speakers while operating forklifts and electric powered jacks (MPA, P1, P2). 7. Management is not enforcing refresher training for forklift operators that are involved in near misses, accidents, or witnessed driving in an unsafe manner. 8. Employees are exposed to struck by hazards, in that, employees are operating powered industrial vehicles with arms and/or legs placed between the uprights of the mast or outside the running lines of the truck. The employee's left foot is outside the running lines when using the sit down tugs because the seat can't be adjusted. 9. Employees are driving on dock plates before they're properly placed inside the trailer to open semi-truck doors using the forklift tines when they're frozen shut. 10. Employees are exposed to fire hazards from blocked fire extinguishers in the following locations: MPA - Post A1 by the 185 belt & post F1. P2 - Post M1 (by the water fountain), Post M5, and Post W2. 11. Employees are exposed to struck by hazards from utilizing a forklift to push multiple gaylords in the MPA from bay 128 to bay 101. 12. Forklifts and tugs that are red tagged for horn or light repairs are returned to service without being maintenanced. 2020 Toyota forklift
