Patient waiting room not socially distant. Doors handles/chairs/restroom not being sanitized after each patient. Minimal sanitization in between patients at check in and check out. Not changing scrubs or shoes before/after and shift. Employees break room not socially distant, or masks worn by employees. PPE supplies reduced at the front desk due to cost.
