3410 Douglas Road, South Bend, 46635 Indiana, United States

Patient waiting room not socially distant. Doors handles/chairs/restroom not being sanitized after each patient. Minimal sanitization in between patients at check in and check out. Not changing scrubs or shoes before/after and shift. Employees break room not socially distant, or masks worn by employees. PPE supplies reduced at the front desk due to cost. Cavi wipes replaced with alcohol wipes to be used sparingly. Level 3 masks replaced with level 1, employees asked to use 1 per week.
