830 Chalkstone Avenue, Providence, 02908 Rhode Island, United States

5th floor administrative areas including room # 515, Specialty Services : 1. Four employees work in a small space that is poorly ventilated. The work involves close interaction between these employees and other employees who come and go from this space through out the day. The four employees are not required to wear masks while in this space, and there are no barriers between them. While their work stations are 6 feet apart their work often brings them with in 6 feet of each other. 2. When one of the workers tested COVID 19 positive this week there was no notification of, and no attempt at a COVID 19 exposure assessment of the other workers. 3. The work area where the COVID 19 positive employee worked was not cleaned and sanitized after the worker was removed.
