1. There is a men's house and a women's house at this address. A male has tested positive for Covid-19. He was not quarantined between getting the test and the positive result. It is reported that he is now being quarantined in an entry area of the
building. 2. Residents all eat together and use the same bathrooms. Groups have been conducted without wearing PPE and they are not 6 feet apart. PPE was not provided until the day this male tested positive. Staff was not provided shields until the shields arrived around May 13, 2020. 3. Residents were not provided any type of face masks or shields in the women's home and men were provided homemade face masks about a week and a half ago for appointments outside of the treatment center
