Employees towards the back corner of the production area are not required to wear face shields or safety glasses while grinding metal automotive housings. 6. Improper guards or no guards on presses. 7. Presses able to be activated with one hand. One press has levers on it that can be activated with one hand. If employee holds the levers down to long, hot hydraulic oil sprays out the hoses on top. 8. Steel toe shoes not required. 9. Improper or non-functioning blue lights on fork lifts. 10. COVID-19 protocol not being followed. Air driers encouraged to be used in bathrooms, paper towels not being supplied.

See Morerklift operators are not operating forklifts safely (not signaling when coming around corners or positioning the height of the forks properly while traveling). 5. Forklifts are being operated outside on uneven surfaces that cause the forklift and the loads to become unbalanced. 6. Razor blades with their blades exposed are left out unattended in high traffic areas. 7. Floors in the work areas are covered with water, dust and debris from stone cutting operations causing slip and trip hazards. 8. Floor drain grates in working areas are not maintained, many are bent upwards causing trip hazards . 9. Employees are not being provided with adequate PPE including steel toed boots, masks, respirators, eye protection and hard hats. 10. Employees working with a white powder additive that is added to a water tank are not provide with adequate safety precautions including no respiratory protection, no eye protection, no training on the safety and health hazards of the additive, and have no access to the additive's SDS. 11. Defective overhead cranes are being operated. 12. The employer does not enforce the use of masking to minimize the spread of COVID 19. 13. There is no cleaning of high touch contact areas as a means of minimizing the spread of COVID 19. 14. There is no process to limit employees who have tested positive for, who are at risk of contracting, or who are showing signs and symptoms of COVID-19 from coming into work.

“In the Chinook Building on the 7th floor, there are multiple COVID 19 safety hazards: 1) Onsite CSS employees have been very lax about wearing their masks while in the Chinook Building 2) PPE is provided on small table near the front door but there is no signage indicating what it is for or how to u... See Morese it 3) There are no Plexiglas or any other type of barrier separating the cubicles. 4) The food area in the NW corner of CSS is not cleaned after use. 5) There are several open boxes of used phones (~50 - 100) with no information regarding whether the phones have been properly sanitized. 6)No wipes, hand sanitizer or gloves are available in the printer/copier room 7)Drinking fountains are fully operational 8)The barrier between the urinals in the men's restroom is too short; the sinks have no additional barrier, and there is no signage regarding occupancy. The sinks and urinals are <6' apart. 9)There is no social distancing in the lunch room. Four chairs at each of the 3 round tables are less than a foot apart. 10)One office has been converted into a conference room with a small table and 6 chairs about 2 feet apart. There is no signage indicating occupancy limits here or in all other conference rooms on the 7th floor.

“I ordered a "Battery-Powered Wood Cutter x 1". The picture of my order was very similar to the one next to this report I am making. They sent me an email back telling my they had two options on the page and I chose to pay for some chain looking thing that looks like you can kill someone with it. And... See Morethey refuse to give me my money back. I have the order, email correspondence and photos of the product I ordered. They advertise in very large photos one thing and then have a very small photo in the corner of something else. You had no idea was even there. Until you don't get what you ordered, but in stead you get the tiny thing in the corner. I can supply all this to you if you can stop these thieves from continuing there false business practices.
