“There is a person who knows that they have Covid 19 as they have been tested and has made this public to others in the area where he lives. There are approximately 5 to 6 people living with HIM in the same house and they all have Covid as they have all been tested and although this individual knows... that he has the Covid 19 disease he still goes out during the STAY AT HOME ORDER and he goes to work daily using the Hamilto Public Transit. He was observed getting on a City bus this morning Saturday April 10, 2021 at the corner of Wentworth Street and King street east at approximately 6:45 am. The Bus number was #1434 which is a double length bus. This person is described as being approximately late 50's to mid 60's in age. He was coughing into his hands with no gloves on and then got on the city bus and was grabbing the vertical bars and seats. This person has made it public that although he has Covid 19 he still goes to work daily a that his boss knows he has Covid but allows him to still come to work. This person knows that there is a STAY AT HOME LOCKDOWN ORDER and DOES NOT CARE AT ALL for the safety OF OTHERS HE IS COMING IN CONTACT IH. his is a very serious situation and it is obvious that this person has spread his Covid 19 disease to several others while he is out in public. This needs to be dealt with immediately. ”

“1. No COVID 19 supervisor 2. Employee's taking own temps, all handling the tracking clipboard 3. Some of the employees in the warehouse aren't wearing masks 4. Mask use communicated out but not enforced 5. Management allowed an exposed employee to come to work, the employee testing positive on 12/29... /20. Only a memo was circulated to most staff about it on 12/30/20, others having to ask for a copy. 6. Employees congregate together in break room, including a pizza party last week 7. Mixed nuts and popcorn are set out for anyone to reach in and grab - may have been resolved only as of 12/30/20

“Injection was on my left arm. Felt a little tingle on my right hand. Fortunately, other than that I didn't feel anything else.
For those of you heading there, the entrance is on the other corner of the parking lot. I went in the late afternoon, there weren't many other vehicles. They've set up co... nes and there are people there to direct traffic. They definitely planned for more vehicles, as I had to make at least 4 turns up and down the parking lot to get to the injection site.
After receiving the shot, you'll be asked to wait for about 15 minutes to make sure you don't have any severe reaction. I actually didn't stick around that long. Take your time and remember to book your next appointment while waiting. Flag the attendants for the QR code.
No cameras allowed. Young'uns, we know you're healthy, please don't be taking selfies and blasting your cheap stereo. Be considerate to others who may be anxious about their health and reaction to the vaccine. Good luck! ”

“60-100 people on busy nights in approximately 100 square feet of space in "High limit" section of the casino. Employees jokingly call this area "Covid Corner". Patrons, while "required" to wear masks, are not asked to leave when refusing despite the casinos "Zero-Tolerance Policy". Guests are in clo... se proximity to both employees and each other. No hand sanitizing is required, stations are hard to find if available at all. Numerous employees have now been infected with coronavirus. On 7/13/2020 management in Table Games were made aware that a dealer was visibly ill with multiple symptoms of coronavirus. He had called out the previous day for flu like symptoms. He was permitted to work the full 8 hours, coughing and sneezing while interacting with guests. On 7/14/2020 human resources made phone calls to numerous employees to inform them that NAME PROTECTED has coronavirus, you should get tested, you will not be permitted to return to work until you provide negative test results". This violates OSHA and HIPA requirements on confidentiality. Approximately 2 hours after these calls went out the entire casino was aware of who was being asked to be tested and that NAME PROECTED had contracted it. I've had numerous sick players and been told to "stay quiet" because they are playing large amounts of money. Employees, including cooks while working, are not wearing their masks. Once in employee only areas, most employees remove their masks completely despite several being confirmed to have coronavirus.

“T8CCR §3203(a) The sheet metal on the frame for the shelves inside the freezer has sharp corners and nothing to protect employees from getting cut. The handles of the cart used to ferry trash out to the dumpsters are broken. Employees do not wear their masks correctly. T8CCR §3270 The employer does... not provide a ladder to allow employees to gain height so they can pull product from freezer shelves. T8CCR §3273(a) The employerdoes not provide a proper floor mat in the kitchen. Instead of a rubber mat, a wooden pallet is placed on the floor of the kitchen. T8CCR §3380(f) The employer does not provide eye and face protection for employees who empty hot oil from the fryer. T8CCR §3384(a) The employer does not provide sufficient hand protection for the handling of hot cookware. T8CCR §3400 There is no first-aid kit or burn kit inside the kitchen.

“1. The fire extinguisher in the guard house has been removed. 2. Staff have to step off curbing and stand against trucks when they come into the facility. The slope of the curbing drops quickly and the space between the curb and truck is tight. Employees can lose their balance and fall when stepping... off the curbing. In addition, some of the drivers do not set the brakes on the trucks and they could move when the security staff is next to the truck. 3. The microwave in guard house is in the bathroom, which is unsanitary. 4. There are no North Carolina Labor Laws posters at the guard house. 5. Cleaning chemicals are stored all over in the bathroom of the guard house. 6. The guard house if very small and measures about 6 feet by 4 feet. There are up to three employees in it at one time and the employees cannot maintain a social distance of 6 feet for COVID-19. 7. Hand sanitizer is not always available. 8. Face masks are locked in a cabinet in the bathroom and are not always accessible to employees. 9. The interior surfaces of the guard house are difficult to clean for COVID-19. The windows have bubble wrap against them, which cannot be cleaned. 10. The golf carts used by employees are stored behind the guard house. The area does not have lighting and employees have tripped and fell. 11. Some of the golf carts have shorted out when charging. There is concern that soemthing is wrong with the receptacle used to charge the golf carts. 12. There are no windshield wipers on the golf carts when driving them in the rain. 13. Some of the golf carts do not have headlights. 14. The brakes on the golf carts sometimes do not operate. 15. Electrical wiring in the guard house bathroom is exposed. 16. The air conditioning vent in the guard house is clogged and water drips. 17. There is a light on the corner of the warehouse that is supposed to flash when trailers are coming through the area, but it does nto work at times. This exposes the employees to being struck if they are unaware of the trailers. 18. Employees have to enter the "pump house" and the lights do not work at times. There are also insects and spiders in the pump house. 19. There is no fire extinguisher at the diesel fuel tank in the event that there is a fire. 20. The toilet in the guard house backs up a lot. 21. The doors to the electrical panel boxes behind the guard house do not close properly sometimes. One of the boxes has a wire hanging down with tape on it. 22. The box used to keep the chargers gets wet and employees could receive an electrical shock. 23. The electrical outlets in the guard house that are used to charge phones have shorted. 24. One of the electrical outlets in the guard house has a cracked faceplate. 25. Paint is flaking off the walls in the guard house and there is concern that it could be lead-containing. 26. The refrigerator under the desk in the guard house sometimes puts out static electricity. There is concern that the wiring may be incorrect. 27. There are roaches in the guard house and there is no pest control program in place.
