each use. There's no social distancing in the cafeteria. Employer is not enforcing the proper use of the mask. 3. Lockout/Tagout was not used while removing plastic from the mold inside Injection Molding Press 1 located in Building 1 on July 2020. 4. On October 2018, a forklift tipped over while positing an injection mold into Injection Molding Press 1 located in Building 1. 5. Pallets of boxes fell over when a forklift operator improperly stacked them in Southwest corner of Building 2 on February 2019. Boxes came close to falling onto an employee. 6. A forklift driver struck and pushed a pallet into rework tables located in the Polish Room at Plant 1 on January 2020. 7. Forklift operators are not using their seatbelts and speed throughout Buildings 1, 2 and 3. 8. A forklift rolled down the ramp located between Buildings 1 and 2 and struck a guardrail on June 2018. The operator did not set the parking brakes. 9. The temperature control system for the tank containing solvent based clearcoat malfunctioned located behind Paint Line in Building 1 on June 2020. 10. Side cutters used to cut 3/8' of ABS plastic is causing blisters to employee's hands and wrist problems located at Press 1 in Building 1. 11. Angle grinder was not guarded in Maintenance Area. An employee cut their thumb on November 2019. 12. Roofs are leaking in Building 1 causing slip hazards.

from 06-01-20 to 06-04-20. Employees are concerned because they work next to the two employees. They also handle every package, every part, and handle every item in that department that they touch. Management is not telling when the two employees caught the virus. (2) The company did not do anything about cleaning or sanitizing anything in the factory when they was told that several employee tested positive for the virus. Just because the employee that tested positive was quarantine does not that everything item or everything they touch in that factory was not contaminated. There approximately 300 employees that could possibly be exposed.
