Employees are not required to wear face coverings at their desks, even though employees are not situated at least 6 feet apart and cubicles are separated by porous sheets of cloth instead of tall, solid cubicle walls or plexiglass. The cloth partitions are short enough to see/cough/sneeze over. 3. All employees are using the same thermometers and pens to take and record their temperatures upon entry with no supervision. The company does not require that pens and thermometers are sanitized between uses, and employees can lie on their reports because no one is monitoring the entry process. 4. An intern who worked in the office had a confirmed case of COVID-19, and employees found out through a grapevine instead of a topdown email. Workers who were in the office with the infected intern were not required to get tested or quarantine for 14 days. 5. The company has not offered or required any training for COVID-19 workplace practices and protocols. Employees were not informed of a response plan in case of a confirmed infection in the office. 6. There are no rules for cleaning/sanitizing equipment or areas that many employees use such as copy machines, printers, kitchen counters, refrigerators, the toaster oven, and the ice machine. The office is still offering coffee from communal coffee makers to employees. The ice machine has black mold growing on the inside of the door, so it's obvious it is not cleaned regularly. Communal refrigerators contain old food and are still in use. 7. The company is not promoting remote work, and some supervisors are using returning to the office as a threat to employees and departments, implying disciplinary action if employees do not return to the office or saying they will require workers to return to the office when something doesn't go the way the supervisor wants.

In-office intern tested positive and in-office employees were not required to get tested or even leave the office. Not all staff were informed. 6. Preparedness and response plan not available to staff. 7. No Covid-19 trainings available. 8. No physical barriers. 9. High-touch surfaces (i.e. kitchen and copy machines) are without protocol. 10. Dedicated entry point is right next to a pod of 4 desks. 11. No response plan for confirmed infection in workplace. 12. Not restricting travel at all. 13. Have not adopted additional infection-control measures. 14. Not promoting remote work, not accommodating doctor's notes for pre-existing conditions (i.e. compromised immune system) and monitoring employee social media accounts for doing things outside the home.

harassed by management and the office to come in regardless of their concerns for their health and safety; although they are getting work done perfectly fine remotely. 3. Desks cannot be moved the minimal 6 feet apart. 4. The check in desk that is not monitored has been moved to the lobby. 5. There is still no Plexiglas between desks. 6. There is no sign of sanitation wipes. 7. There is no process regarding an outbreak in the office. 8. The icebox still has what appears to look like mold. 9. No designated walk paths to keep workers apart from each other. 10. Employees told they do not have to wear masks at our desks, only in "common" areas such as the kitchen. This does not make them feel safe. The office is an open office layout because there are no cubical or strong barriers, but closed because employees cannot open any windows.

Chloride Irrigation USP and Sterile Water for Irrigation USP. The Nurse Assist recall was issued due to the potential for a lack of sterility, which could result in non-sterile products. Medline Industries, LP has received one adverse event associated with product manufactured by Nurse Assist.

Water-based medical products that are non-sterile and potentially contaminated could cause serious or life-threatening infections, including bloodstream, urinary tract, open wound/soft tissue, and respiratory infections. Patients who are elderly, critically ill, have weak immune systems (including newborn infants, pregnant women, and cancer patients), or have chronic diseases are particularly at risk of infection. However, other patients could also develop infections after exposure to contaminated water-based medical products.

The recall affects Medline and Centurion convenience kits that may be used at home by patients, caregivers, and home health providers, as well as convenience kits that may be used in medical settings for surgery or other medical procedures. Recalled kits include, but are not limited to, wound care kits, tracheostomy kits and catheter kits. A detailed list of recalled Medline and Centurion convenience kits, as well as an example of product labeling, can be found in the link below.

Consumers who have recalled product should stop using the product and contact Medline immediately.

Medline Industries, LP notified its direct distributors and consumers of the Nurse Assist recall by First Class Mail and email on 11/16/23, upon receipt of initial recall notification from Nurse Assist. Customers who received the recalled product were given instructions to quarantine all affected products immediately and provide Medline with a response regarding the affected quantity on hand. Upon confirmation of the affected quantity, Medline provided customers with over-labels to place on the affected inventory, with instructions for staff to remove the affected component before using the kit. The recall notification also instructed distributors and those who resold or transferred affected product to another company or individuals to notify their customers of this recall notification.
