was sent home. Rexair didn't clean or sanitize the line until at least 30 minutes after the incident. They started cleaning after people were talking about reporting it. The company did not adhere to Center for Disease Control (CDC) recommendations for cleaning. This situation put everyone on the main production line at risk. 2. There are office people that walk around without facemasks covering their nose and/or mouth. 3. Not sure if the company treated the fecal matter/vomit as a biohazard, or if they just tossed it in the trash. In addition, general safety/health hazards were also identified: 4. The unsafe practice of stacking wood pallets over six (6) feet high creates an unstable load possibly resulting in pallets falling over and injuring workers.

The plant manager and his manager have been made aware of the laws and the fines, they said no working from home and have made no effort to better their response to COVID (office employees buy their own hand sanitizer, lysol wipes, etc). 2. Warehouse racking in plant isn't secured to the floor. Warehouse racking in external warehouse is not secured to floor with various trip hazards. 3. Cardboard is used as covering/safety guards. 4. There are no cut (kevlar) gloves in use at trimming stations on the plant floor. 5. There are unlabeled bottles of fluid on the floor.

NO employees were told they they were considered close contacts regardless of the fact that the department rotates between stations, handles parts that each and every individual has the potential to have had contact with, and the mask order is either ignored or not enforced. Coupled with the fact that the entire plant has been working overtime, typically in that department or another. 2. There have been other employees that have been coming in to work exhibiting COVID-like symptoms regardless of the fact that our questionnaire states that you must stay home if you are.

Per the safety alert update on 3/12/24, "The FDA continues to receive adverse event reports related to the products identified in this Safety Alert."

The recall was the result of a routine sampling program by the company which revealed that the finished products contained the yellow oleander. The company has ceased the production and distribution of the product as the company continues their investigation as to what caused the problem. Further the company is conducting comprehensive quality control and monitoring process.

Customers who have purchased these products should stop using them immediately and return them to the point of purchase for a full refund.

Ingestion of yellow oleander can cause neurologic, gastrointestinal, and cardiovascular adverse health effects that may be severe, or even fatal. Symptoms may include nausea, vomiting, dizziness, diarrhea, abdominal pain, cardiac changes, dysrhythmia, and more.

If you are experiencing symptoms after eating this product it is important to report it. It can help to detect & resolve outbreaks early and prevent others from being harmed, and it enables better surveillance. If symptoms persist seek medical attention.
