safety buttons to stake the spring. I reported it to the team leader and nothing was done, I got the attention of a maintenece man and showed him the safety issue. Maintence refused to fix it. On Wednesday May 18, 2022 the Supervisor and I told him about the safety issue. He did nothing to fix it as well. On Friday May 20th, 2022 the safety issue occured as it had all week and I risked accidental death or dismemberment to my hand or fingers from the staker after I scanned a part at staion 303 Department B-2.

11/30 update: HR said it wasn't reported but it was reported to maintenance, and the supervisorsand. Yet he told the Govenor safety team that nobody said anything
