Safety hazard at workstation

I worked at the staker station and I reported on Monday May 16, 2022 that the staker came down repeatedly during the wrong time(after scanning the part barcode) rather then scanning the barcode, adding the spring part to the seat part with a wrench, then pressing the... See More safety buttons to stake the spring. I reported it to the team leader and nothing was done, I got the attention of a maintenece man and showed him the safety issue. Maintence refused to fix it. On Wednesday May 18, 2022 the Supervisor and I told him about the safety issue. He did nothing to fix it as well. On Friday May 20th, 2022 the safety issue occured as it had all week and I risked accidental death or dismemberment to my hand or fingers from the staker after I scanned a part at staion 303 Department B-2.

Not following the govenors mask mandate put into place July 27, 2020. Employees are unable to stay 6 feet apart which is why the mask are needed. There have been numerous covid 19 cases in the facility in the last couple months since they started back to... See More work after the state shutdown. The mask are needed for everyone's safety from this virus. Employees are usually walking around and several of them are within a foot of one another. Employees are extremely concerned for the safety of everyone around them. There is no possible way to social distance at this place of employment due to the fast pace nature of the work being done. In many locations employees are working side by side or face to face with no shield in between on lines as well company is not enforcing the mask rule and making covid employees come back to work within 7 days
