1. Facility has mold issues; employer has not kept the facility clean exposing employees to excessive levels of mold and employees are having breathing issues. 2. Employees are not provided with face masks/face coverings. Additionally, a company policy, procedures and/or instructions have not been developed and implemented
along with employee training to prevent exposure to Coronavirus (COVID-19). 3. Employees have tested positive for Coronavirus (COVID-19) and the facility has not been cleaned or disinfected in accordance with CDC Guidelines; the company has not developed and implemented policies, procedures and/or instructions to prevent Coronavirus (COVID-19) exposure. 4. Social distance policies including risk assessment procedures have not been developed and implemented related to COVID-19.
