1. T8 CCR 5199(c)(5) The employer failed to implement and maintain effective procedures to reduce the risk of transmission of COVID-19 during the period the COVID-19 case is in the facility in the following ways: - The employer is housing COVID-19 positive cases in the "green zone"
of the facility, with no clear indication that these rooms contain COVID-19 positive cases. Additionally, COVID-19 positive cases have been observed leaving their rooms to enter the hallways in these areas. - The Director of Nursing and Infection Prevention is violating company policies and procedures by failing to sanitize their hands and wear gloves and face shields when entering the "red zone," rooms containing COVID-19 positive cases, and when collecting nasal / nasopharyngeal swabs.
