1. The employer has not identified and implemented necessary workplace controls (engineering, administrative, work practices) to reduce employee exposure to the COVID-19 virus per OSHA publication # 3990-3 2020 and CDC guidelines. 2. Employees have acquired, and are exposed to, positive COVID-19 employees and residents. Adequate infection control prevention policies and procedures are not being implemented and enforced to prevent the spread of the COVID-19 virus. 3. Employees that are showing signs and symptoms related to COVID-19 and/or have been exposed to a positive COVID-19 employee and/or resident are required to keep working. 4. Administrative and work practice controls are not being implemented to reduce employee exposure to COVID-19 (i.e. social distancing, screening staff/residents/visitors, reducing flow of people into the facility, return to work policy, changes to sick leave policies, etc.). 5. The employer has not identified specific type(s) of personal protective equipment (PPE) (i.e. gloves, masks, respirators, etc.) that need to be available and worn to reduce employee exposure to the COVID-19 virus. 6. Employees performing cares and aerosolizing procedures with COVID-19 positive residents are wearing inadequate PPE per CDC guidelines. 7. The employer is not addressing social distancing and infection control prevention as communal dining /activities are taking place with groups of residents and staff. 8. Employees that have potentially been exposed to COVID-19 positive staff and/or residents are not being tested, isolated, and informed of exposures to prevent the spread of the virus. 9. The employer is not providing employee training on COVID-19 infection control procedures per CDC and OSHA guidelines. 10.
