1. Emergency exit doors blocked by freight materials throughout the building. 2. Employee(s) exposed to fall/trip hazard due to aisleways being covered in debris throughout the facility. 3. Inoperable and uninspected fire extinguishers located throughout the facility. 4. Employee(s) exposed to unsanitary working conditions due to rodent infestation in the office, dock platform , yard and perimeter of building. 5. Large potholes located in the yard prevents trucks from operating safely while in the facility. 6. The employer does not maintain an effective Infectious Disease Preparedness and Prevention Program for Covid-19. Safe distancing is not observed and enforced.
