1. COVID-19 rules and regulations are not practiced. There is no social distancing being practiced in the dispatch area, driver break room, and the administrative office. 2. Numerous dispatchers contracted COVID-19 and the business stayed open. 3. PPE masks and barriers are not being used. The mechanics in the shop area refuse to wear PPE. 4. Employees are not informed of positive tests in the facility. 5. Numerous company drivers all tested positive for COVID-19, yet the slip seat truck(s) are without proper sanitization and deep cleaning. 6. Positive cases are not reported to the state. 7. Numerous safety violations in buildings 1 & 2. The electrical outlets by all of the cross-dock overhead doors are missing covers. 8. Junction boxes have exposed wires. 9. The propane tanks are not mounted properly on the powered industrial trucks. 10. New employees are not trained or certified on the powered industrial trucks. 11. No tire wheel chocks or safety jacks for the majority of the dock doors in both buildings 1 & 2. 12. Mechanics are not using jack stands(s) or tire cage when changing tire(s) or filling up truck tire(s) 13. Building 1 has numerous gas cans and flammable aerosol cans out in open areas. 14. Empty propane tanks are being stored inside buildings 1 & 2. 15. Gas cans are stored inside building #2 16. The stair case for the mechanics main office in the shop area has different measurements for the risers and treads creating a trip hazard. 17. The railing at the top of the stairs in not tall enough or secured. 18. There is a second floor built on top of the office in the garage. The wrong truss hangers were used for the load bearing ceiling above employees heads. 19. A few exits at both building(s) 1 & 2 have some type of obstruction blocking the way- in case of fire or emergency to exit. 20. In building #2 the fire extinguisher(s) are blocked with product/material in the warehouse. 21. Product on shelves in buildings 1 & 2 are a potential fall hazards to employee. There are no safety chains or bars used to prevent falling objects. 22. Numerous trip hazards throughout both buildings 1 & 2. 23. The MIOSHA 300 records are not accurate. The worker's compensation claims do not match the logs.
