1. The employer is not enforcing COVID-19 state and CDC recommendations during the pandemic. 2. Employees are not required to wear masks even when they are in close proximity to coworkers and customers. 3. Employees are allowed to work when they are presenting symptoms of COVID-19, such as cough and fever. 4. The employer does not provide hand sanitizer. Many days there are not soap and paper towels in the restrooms. 5. The employer has not notified employees when there is a COVID-19 case in the facility. The employer threatens to fire employees for taking time off to get a COVID-19 test or to quarantine if they are exposed to someone who has COVID-19 or test positive themselves. 6. There is a mouse infestation in the building and the employer refuses to have an exterminator come to treat the facility.Alleged Hazards: 5, Employees Exposed: 50
