Located at the City of Chesapeake, Chesapeake Public Library, 1. Employees are becoming infected with Covid-19 (Greenbrier Library closed); 2. The employer is not taking employees who are at high risk for fatality from exposure to COVID-19 into consideration; 3. No hazard risk assessment of job tasks for library staff; 4. The employer is not making hazard risk assessments available or asking for employee input; 5. No PPE despite library employees handling items that have circulated in the community; 6. No health screenings for Covid-19; 7. Masks are requested of the public in the library but not required by the public despite EO 63; 8. An employee was exposed 6 days prior to an employee's hospitalization with Covid-19 and was not notified of exposure; 9. An employee tested positive for Covid-19 and city management was informed on 7/28/20 but co-workers have never been informed of an employee's positive test, or their possible exposure within 24 hours of notice to the employer of the positive employee as required by the new VOSH standard; 10. Suspect that positive workplace cases are not being reported to the VA Dept. of Health as required by regulation; 11. Employees were not informed of possible workplace exposure on 6/24/20 after an employee reported a positive test from a family member they had close contact with and were on-site working with other employees 2 days before - City management placed this person on quarantine but did not inform employees of possible exposure; 12. An employee became sick with 5 Covid-19 symptoms approximately 2 weeks after this workplace exposure and reported to management that they believed they had a "summer cold". The employee was out of work sick for several days; 13. Management never notified employee(s) of possible Covid-19 exposure and employee was allowed to work while manifesting symptoms of Covid-19; 14. To date, no one at the City of Chesapeake has ever notified employees of occupational exposure, or requested employees be tested despite their knowledge that employees were exposed to an employee in the workplace not just once, but twice who tested positive for Covid-19 and was hospitalized for over 3 weeks and who has still not returned to work; 15. Emergency Temporary Standard (ETS) training from employer Human Resources does not cover the process for employee reporting of positive tests, and potential notifications, how made, and by who as required within 24 hours.Source: Osha.gov | Receipt Date: 2020-08-24
