1. The employer has an established policy that patients symptomatic for COVID-19 go into an isolation room for testing, but it is not being followed. Patients with fevers and symptoms were not isolated unless they were going into another private room for another reason or if it was the only seating available. It has been commonplace to have the patients in the open pod to test for COVID-19. Patients have been treated in the outpatient treatment room while being sick with COVID-19 and staff as well as other patients were exposed to the virus. 2. Employees have not been wearing face masks while in the employee hallways, locker room and break room. 3. Social distancing practices have not been followed. There have been many employee gatherings, including baby and bridal showers, that many employees attended and were close together. There were no modifications made to the break room to allow for distancing. Extra chairs were actually added so several employees could sit together. 4. Employees have removed face masks while in the pods and private rooms in order to drink and eat. It is against the employer's policy to have food and drinks for staff in the treatment pods. 5. Employees have never been checked for fevers or symptoms of COVID-19. Employees walk into the facility sometimes not wearing face masks. A web link for employee screening that was developed does not involve temperatures and is totally voluntary and not monitored by the employer. 6. Multiple employees tested positive for COVID-19 in October 2020 and the employer did not do any contract tracing. No employees were notified that they could have been exposed.
