1. Employees are given one mask to wear for a whole week. 2. Triage and MA rooms are not sanitized. 3. There is no 6 feet distance among patients in the waiting room. 4. Staff is told not to enforce the mask rule to patients. 5. Employees are lacking protective gear, sanitization wipes and only have monthly access to it. 6. The glass installed for front desk is not isolating employees from patients. 7. Shared items such as clipboards are not sanitized. 8. Staff are only allowed to get level 2 masks. 9. Patients are allowed to walk into the rooms with as many people as they want instead of limiting it to patient and legal guardians only. 10. Staff has to buy their own materials. 11. Call center and front desk staff sit shoulder to shoulder and use the same telephone devices. 12. The janitor is the only cleaning person they have except for Saturdays when a cleaning crew comes in. 13. The workflow is so fast and chaotic that staff barely have time to sanitize their own area and change their gloves. 14. Blood pressure cuffs, blood glucose devices and pump downloads are not sanitized at all. 15. The facility was not shut down or cleaned professionally after an employee tested positive. 16. Not all staff was informed that an employee tested positive. 17. Staff is being tested for COVID by someone using regular q-tips and not qualified to perform the testing. 18.
