1. Employees are exposed to COVID-19. The employer does not ensure that all employees, including all managers, wear a mask continuously throughout their entire work shift.2. Employees are exposed to COVID-19. The employer does not ensure that replacement personal protective equipment (i.e., gloves and masks) is always available, for each employee on each shift, as needed.
3. Employees are exposed to COVID-19. The employer does not provide enough hand sanitizer, for all employees on each shift, to use as needed.
4. Employees are exposed to COVID-19. The employer does not ensure that social distancing is practiced between employees and store managers.
5. Employees are exposed to COVID-19. The employer does not notify employees of their potential exposure to COVID-19 after a confirmed positive case has been identified in the facility.
6. Employees are exposed to COVID-19. The facility is not cleaned and sanitized as recommended by the Centers for Disease Control (CDC) after an employee has tested positive for COVID-19.
