1. General Contractor is providing training to employees in the new hire orientation that contradicts OSHA standards when telling employees (craft workers): a. that scaffold only has to be inspected every few weeks, b. allowing workers to remove guards, c. excavations aren't being inspected by a competent person, d. broken fall protection is being used by several subcontractors. 2. An effective Infectious Disease Preparedness and Response Plan for COVID-19 has not been developed or implemented, including but not limited to, performing temperature check and wellness screening. 3. House keeping issues throughout the site to include but not limited to; nails, metal scrap, wooden stakes and debris that could cause injury to employees walking and or working around such areas. 4. Phase 2 of the building site, employees are not using fall protection when exposed to fall hazards while installing structural steel. 5. In the QE building the ladders used to looked into the water tanks are zipped tied to the tank, employees are required to use the ladder on a daily basis to inspect the inside of the tower. 6. Employees are working in ceiling areas that are not finished and are not wearing fall protection. When employees do wear fall protection there is nowhere to tie off within the ceiling area. 7. in the pump room employees are required to stand on the piping to open the valves to fill the water towers. Employees are exposed to possible slip and falls, struck by hazards from tools used to open the valving.
