58 Meetinghouse Lane, Bourne, 02562 Massachusetts, United States

Facility: 1-Work place is not routinely cleaned to help minimize work place dust and trash. 2- Upload Area: Main heater is not operational. Employees working in cold temperatures. 3- Not all employees are covering their mouth and nose with a face mask to help minimize the spread of COVID-19. 4. Employees including management, have not been trained to keep their mask on while talking. 5. Work operations have not been set up to ensure at least six foot distance in all directions form other workers. 6. Several employees may work inside a trailer within six feet of each other and they don't wear their mask. 7. There have been COVID-19 positive employees and not all employees who have had close contact, have been informed on current quarantine, testing, and return to work protocols.
