The employer did not maintain an effective Injury and Illness Prevention Program in that the employer did not ensure that COVID-19 precautions per the state executive and local public health department orders are being followed at the workplace in the following instances: 1. The employer did not perform a detailed risk assessment and implement a site-specific protection plan. T8 CCR 3203(a)(4). 2. The employer did not train employees on how to limit the spread of COVID-19, including how to screen themselves for symptoms and stay home if they them. T8 CCR 3203(a)(7). 3. The employer did not implement individual control measures and screenings. T8 CCR 3203(a)(6).

stop any further activity with this entity. I found a phone number for the company Subgenix Products in the bottle. 800-609-4174 and called it. The receptionist answered in a foreign language which sounded oriental and I also heard what sounded like Spanish. When I requested an English speaking person to assist, they hung up the phone. I called again with the same results.

day as I watch cable television I scanned the QR code as it says, and the companies name is supposed to be bright fire. Advertising a rechargeable, USB ported, electric plasma, lighter that requires no fuel, only electricity I got a cheap China torch lighter in the instructions are in the size of a credit card and it is all in Chinese writing.
