A ring from GRA Moissante I did not order, Oregon, USA

I received a ring and an envelope containing a card with a scan bar in it from GRA Moissanite I did not order. Don't scan the QR code. The return address is from Cody Polk 10175 Harwin Dr. Set 100A Houston,TX77036 When I looked this up several websites stated this was a scam.
