referral: Lift-platform in warehouse in disrepair, no training, unsafe at height. Packages not stored on warehouse shelving for safe removal or stability. Dry Ice sales no SDS on Dry Ice and no thermal protection while transferring the material to containers. Throughout building in office, clock-in area, and break room- Employee has concerns regarding the potential exposure to COVID-19 due to the following: Employee alleges that employer is not enforcing the wearing of masks while in the presence of others. 6-foot social distancing not being adhered too or enforced by employer. No safety officer on site to have oversite of COVID-19 policy and procedures. No prescreen questionnaire or temperatures being taken prior to entry. Employee alleges that employer did not provide training until 4/10/20 and is unsure if they have trained all employees and have kept a record of training.
