warnings. 3. An employee complained of back pain, and the employer wanted to send this employee for a drug test, which discourages injury reporting. 4. Pallets are crooked on the racks and could get struck by other pallets getting placed onto or removed from the racks, especially on the high rack levels. This could result in the pallet or boxes falling and striking employees below. 5. The LED lights are very bright making it hard for powered industrial truck operators to see when placing materials or retrieving materials from fifth level storage areas. 6. There is no social distancing for infection control in the cafeteria, locker room, and when entering to start work. At the start of the work days, about 80 people come in at one time and congregate close together.
