Employees are exposed to the following alleged hazards: 1. On Door 1, the 100 belt sparks during package unloading and is not in properly maintained in good condition. Findings: After thorough investigation, it was found that at one point the 100 belt was sparking during package unloading. … See More
It was found that once this was identified by management, the belt was taken out of commission until contractors were able to come on-site and repair. The 100 belt was not observed sparking and was observed in overall good condition at the time of the inspection. As a result, no citations were issued. 2. On Door 2, the cotter pin is the wrong size so extreme care is needed near the belt so as not to be struck by the cotter pin when pulling it out the or rolling it back in. Findings: After thorough investigation, it was found what was determined to be the cotter pin on the belt by Door 2 was not the correct size. However, it was found that the cotter pin did not present a hazard to employees. The employer repaired the cotter pin and ensured the proper pin was installed on the belt by door 2. No citations were issued. 3. Belt 3 is not maintained in good working condition and typically does not work but when it is in service it is very loud, and emits a burning smell. Findings: After thorough investigation, it was found that Belt 3 was previously emitting a loud sound when in operation. However, it was found that once this was brought to management's attention, a contractor was called in to conduct repairs of the belt. At the time of the inspection, Belt 3 was observed in operation and was found in good working condition with no loud noise or burning smell. As a result, no citations were issued. 4. The employers require employees to lift heavy boxes individually instead of the designated team lift process. Findings: After thorough investigation, it was found that the employer did not require employees to lift heavy boxes individually. It was found that the employer had a team lift process and policy in which was communicated with employees and implemented facility-wide. It was found that supervisors encouraged and reiterated the importance of utilizing the team lift process. Signage was observed throughout the facility reiterating the importance of team lift. The employer was found to enforce this work rule with employees. As a result, no citations were issued. 5. Pallets are stored on their sides in a manner that causes trip and fall hazards. Findings: After thorough investigation, it was found that rarely pallets were stored on their sides. It was found that these pallets were identified by employees and managers alike and corrected immediately. The employer was found to have a policy and work rule on safe pallet storage that was found to be communicated to all employees. The employer was found to inspect the workplace regularly for this hazard and abate as necessary. At the time of the inspection, no pallets were identified stored in an unsafe manner. As a result, no citations were issued. 6. Trailer door flaps, and latches are not well maintained and there is very poor housekeeping in the trailers. Findings: After thorough investigation, it was found that at times, there were damaged door flaps, latches, and poor housekeeping identified in the trailers. The trailers were found to be managed by a different company and when these issues were identified, the employer was found to have a system in which these trailers were "red-tagged" and removed from service if any safety issues were identified. The employer was found to communicate any issues with housekeeping to the service center supplying the trailers. At the time of the inspection, no damaged trailers or trailers with housekeeping issues were identified. Numerous trailers were observed red-tagged and stored in the parking lot out of service. As a result, no citations were issued. 7. Safety work gloves were not provided to employees. Findings: After thorough investigatio
