1. Use of improperly diluted industrial grade cleaner without PPE by untrained staff. Cleaning gloves or proper masks were not on site. 2. Product was 2 employees used "degreaser" purple label made by "KAY", one got some on their skin and experienced irritation. 3. Zero social distancing.
4. Little adherence to wearing masks. 5. No hand washing. 6. Knowingly asked an employee who had a potential COVIS diagnoses to come into work.
