1. Currently there are at least five COVID positive cases and the staff is not being informed of such by Management; two hostesses, a bartender, a manager, and a kitchen banquet manager are out with COVID. 2. There are at least two kitchen staff who do not
wear a mask at all; one of them is a prep cook during the day.

1. Masks are not being worn by members of the restaurant and it is part of a country club so members are rude and do not comply. Managers don't request that they comply or speak with them seriously about what they are doing wrong. 2. Countless numbers
of employees are attending work with covid-like symptoms and putting others at risk. 3. Sanitation measures are not strict at all; there is no sanitizing of doors, pens, menus etc. There have been 10-20+ employees at Firerock that have had covid in the last month and the establishment has not paused work for sanitization or contact traced anyone. Besides hearing from other employees themselves, employees have not been updated on who has covid at our workplace or when or if they were in close contact with others.

1. The general manager required an employee to report to work even though the employee had informed them that they were experiencing COVID-19 symptoms (i.e., clear loss of taste and smell, a cough, and other cold-like symptoms) and was not feeling well. The employee continued to work
multiple days with symptoms worsening and eventually tested positive. The restaurant was never shut down and cleaned. 2. The manager doesn't enforce the use of masks in the workplace. Furthermore, a friend of the the manager's came into the establishment and was excluded from having to wear a mask or fill out contact tracing forms.

1. An employee revealed that they had tested positive for COVID-19 and was contagious while working in the establishment. This was reported to the business by the employee, but this information was never communicated to those coworkers who were potentially exposed. Employees aren't able to always practice
social distancing and furthermore they interact a lot with customers during a given shift. Furthermore, there are employees working in the establishment who care for individuals who are considered high risk. 2. An employee who was showing symptoms was required to still work. The employer is not administering any type of check list, and furthermore is not taking any employee temperatures. The employer continues to permit employees to work while sick.
