for COVID-19. That same day the employee went to get tested and ended up testing positive. Again management did not report to staff or clients that someone else had tested positive for COVID-19. Now a third employee has tested positive, and employees still have not been informed, nor has anyone come in to sanitize or deep clean. The positive results could also have been prevented if the Company allowed counselors to work from home using work computers. The counselors go into the office Monday through Friday and call clients on the phone. For those clients that don't have a phone, the Counselors have to see their clients face to face. The Company is also allowing the agency to take intake clients in the building without being tested for COVID-19.

still required to be physically present in the clinic 40 hours a week and do face-to-face intakes and appointments with patients who do not have a phone. The employer has not implemented restrictions for limiting staff onsite and employees are not allowed to work remotely. This violates the "Promote remote work to the fullest extent possible" and "Restrict the number of workers present in-person on the job" sections of the Executive Order. On any given day there are five counselors working, receptionist, supervisor, clinic director, 2-3 nurses, and a part-time doctor. These employees are all required to be present in the building. 2) The receptionist does not wear a mask, nurses ran out of masks, and approximately 100 daily patients are not provided masks. Employees have been reusing the same (disposable) mask for weeks. 3) The patients do not practice the 6-foot social distancing rule. 4) A new patient tested positive for the coronavirus and is currently in the hospital. This patient was in the building for several hours on April 7, 2020. This building is not that big, and this patient was in the lobby, restroom, lab, dosing booth, doctor's office, and a smaller office to do paperwork.

reports of lacerations, two of which required medical attention, including stitches. These injuries occurred during assembly or cleaning/handling of the product.

The choppers were sold at Walmart stores nationwide and online from August 2022 through October 2023, priced between $10 and $15. The recalled product is the Mainstays Electric Mini Chopper with model number MS14100094536S1. These choppers are rechargeable and cordless, featuring a green lid, white body, and clear plastic bowl. They come with a single blade attachment, which includes three curved blades on a white plastic spindle. The model number can be found on the label attached to the bottom of the clear plastic bowl.

Consumers should immediately stop using the recalled choppers, and contact Walmart to receive a full refund. Consumers can bring the chopper to their nearest Walmart store for a refund.
