Location: Warehouse, Level 2 4) Approximately 35 employees are exposed to electrical hazards, in that the electrical wiring for the Crumbing Production Lines are damaged and lying in standing water that is about 1/2 to 1 inch deep as employees work at and around the machinery. Location: Crumbing Lines 1 and 2 5) Approximately 35 employees are exposed to electrical hazards, in that there are exposed electrical conductors that are covered only by rubber gloves. Location: Crumbing Line 1, at the point where batter is deposited 9) Approximately 120 employees are exposed to amputation hazards, in that gears and sprockets on the production machinery are unguarded as employees work at and around the lines. Location: Production Department, Packaging Lines 10) Approximately 120 employees are exposed to fall and struck-by hazards, in that ice is allowed to accumulate on the work floors as employees walk and operate forklifts across them. Location: Production Area 11) Approximately 120 employees are exposed to struck-by hazards, in that damaged forklifts are not taken out of service. Specifically, the lift in the Freezer has damaged/treadless tires and the Yale forklift that is used in the Shipping and Receiving area has unresponsive brakes. Location(s): Freezer and Shipping/Receiving 12) Approximately 120 employees are exposed to struck-by hazards, in that pallets with product on them are stacked about 18 feet high without means to secure them. Location: Warehouse 13) Approximately 35 employees are exposed to health hazards, in that the employer has not implemented an effective vermin control plan where there is evidence of an infestation or roaches. Location: Crumbing Area 14) Employees are exposed to health hazards related to SARS-CoV-2/COVID-19, in that employees are not practicing social distancing while working.
