The crew supervisor did not wear PPE such as a hard hat as required when altering the lift truck. 4. Employees are not following CDC Guidelines to protect employees against the hazards of contracting COVID-19. Specifically, social distancing and the use of face coverings were not used when conducting indoor training. At least two employees sitting next to each other contracted COVID-19 and exposed other employees to the virus.
