Presidio Forestry Crew, Shop : 1. The crew supervisor removed part of the overhead safety guard on the lift truck without manufacturers express permission. The efficacy and integrity of safety equipment has been affected and will not protect employees against overhead hazards. 2. The crew supervisor worked from heights in excess of 10 ft. when altering the lift truck overhead guard without the use of a fall protection system. 3. The crew supervisor did not wear PPE such as a hard hat as required when altering the lift truck. 4. Employees are not following CDC Guidelines to protect employees against the hazards of contracting COVID-19. Specifically, social distancing and the use of face coverings were not used when conducting indoor training. At least two employees sitting next to each other contracted COVID-19 and exposed other employees to the virus.Source: Osha.gov | Receipt Date: 2021-03-04
