- comments: observed evidence of appx. 75 or more mice droppings on floor near storage shelf in prep area,in between and behind file cabinet & safe in office,in lunch room near lunch room tables and floor cleaner. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 9/11/20. (priority foundation 7-38-020(a)

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster lids are missing & damaged and massive amount of boxes stacked on ground around dumpster. Must replace and repair lids and remove boxes from garbage area and maintain. (priority foundation 7-38-020(b)

55. Physical facilities installed, maintained & clean
- comments: must clean debris from floor behind ovens.

SKAM!!!!!!!!! I ordered a cross made out of horseshoes. Pieces were plastic and very small. Not cut right so nothing fit together. Worthless piece of junk! Had to try to put it together with many pieces and it is crooked and warped looking.
