- comments: 4-602. 11(b-e) observed stained cutting boards with grooves. Instructed to clean and sanitize or replace cutting boards.

- comments: 6-501. 111 observed evidence of live small flies activity on site. Observed about 15 small flies located on the ceiling and shelves around the 3 compartment and about 10 small flies located in the rear office by the utility sink. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
