1. Employee in the facility are potentially exposed to COVID-19 due to the employer not cleaning and sanitizing the bathroom areas when necessary. 2. Employees using the rusted eyewash station that does not work properly are exposed to potential burns and other chemical hazards. 3. Employees are exposed to unsanitary conditions in the rest rooms because rest rooms have mold/mildew, missing and broken toilet seats, nonworking dispensers and a generally ill-kept bath room. 4. Employees in the facility are exposed to potential fire hazards due to blocked fire extinguishers. 5. Employer has not created a clear egress plan or marked emergency exits for evacuation due to potential fire hazards. 6.
