1- A parent brought a symptomatic child / infant to the facility after having tested the child for Covid-19. The child tested positive. This symptomatic child/infant was not properly screened before being allowed in the facility. 2- This child / infant was in a room approximately 2 feet from other children /infants and could have potentially spread the Covid-19 to other children / infants, however 5 other children /infants in the same room were sent to 2 or 3 other classrooms, thereby potentially infecting many other children / infants and staff. 3 The staff in the original source room and subsequent rooms were not notified of a positive child/ infants status to alert them to the hazard of other children/infants that were brought into their classrooms. 4- The facility does not have a covid protection plan or emergency operations plan as outlined in the "CDC´s Guidance for Operating Child Care Programs during COVID-19". 5- The facility has not established a designated Covid-19 point of contact for staff and parents. 6- The staff have not received formal training in Covid-19 employee protective policies such as mandatory mask wearing, social distancing, who to report a potential illness or positive status to, duration of quarantine, what a possible contact with infected person or family member is, or the nature and hazards of Covid-19 hazards. 7- The source room was not sanitized.Alleged Hazards: 7, Employees Exposed: 4
