1. The employer has not implemented policies and practices, to limit the spread of COVID-19 in the workplace, such as but not limited to: a. Performing enhanced cleaning and disinfection after people with suspected or confirmed COVID-19 have been in the facility and has not opened doors and windows or otherwise increased air circulation in the area. b. Implement or enforcing workplace policies, such as social distancing, designed to reduce the transmission of illness between employees. c. Enforcing policies requiring workers to wear masks or face coverings. d. The company did not ensure that employees that were exposed to persons who tested positive for COVID-19, were informed and required to quarantine according to the CDC guidance. e. The company did not screen those that enter the building and ensure that those The exhibiting symptoms of COVID-19 were separated from other employees and immediately sent home. f. The company continues to permit employees who have recently traveled from other states (Texas and Florida) to enter the office without following CDC guidance and local travel restrictions which requires quarantining for a full seven (7) days for interstate travel. 2. The company did not conduct a Personal Protective Equipment assessment and provide adequate training for workers in the laboratory, workers wear open toe shoes, and fail to wear eye protection and lab coats while working in the lab. 3. Fine particulate is generated in the foundation lab when grinding pigment but there is not adequate capture or collection therefore the particulates suspend in the air exposing nearby workers to a respiratory hazard. 4. There is not eyewash or shower in the mascara and lip labs despite the use of concentrated materials that can cause injury to eyes and skin. 5. The employee refrigerator and water dispenser are contaminated with batch material due to improper hygiene and insufficient cleaning.Alleged Hazards: 5, Employees Exposed: 65
