(1) Training - Most employees do not have knowledge of or have been trained to follow the Emergency Action Plan. The Emergency Action Plan either does not exist or has not been updated. No Exit map for rear office. No alarm system. (2)Training - Most employees do not know the location of the Safety Data Sheets and have not been trained as to their location. (3) Fire extinguishers are not checked on a monthly basis. (4) Eye wash stations are not located is areas where corrosive materials are present (batteries). (5) Operations - Employees are allowed to ride the rear step of the collection vehicle in speeds over 15 MPH while on roads with multiple lanes of traffic. Employees are allowed to ride the rear step of the collection vehicle while vehicle is in reverse without any fear of disciplinary action. (6) Maintenance Shop - employees are allow to operate power tools with the guards removed. Hand held grinder. Employees are allowed to use high pressure air to blow off themselves and equipment. Air nozzle not fitted with the required end fitting allowing unconstrained air pressure to exit the nozzle. Portable containers are not labeled. (7) Injury reporting - Two management employees contracted COVID-19 while on company business and it was not reported as a OSHA recordable illness. Their illness was kept confidential to employees of the company.
