Employer not ensuring employees wear their mask and sanitize hands when giving n95 and surgical mask at the start of their shift to other employees. Employer not ensuring employees wear their mask correctly (nose out, wearing around next, pulling down mask when speaking, or wearing inappropriate mask). Employer not ensuring employees wear appropriate mask (N95 with valves, neck gaiters, etc.) Employer not ensuring employees wear the masks. Employer not enforcing the use of masks. Employees constantly are taking off their mask around co-workers and patients. Especially in small shared offices without ventilation or social distancing. Some staff members are defying/refusing the use of masks. Employer not providing appropriate face covering to all employees. Employer risking the health of employees by exposing them to Covid-19 infection due to retirement party at the Inpatient Treatment Facility, the inpatient unit of the psychiatric hospital, break room, where employees do not wear a mask, and no ventilation at work party. Employer not conducting temperature check and systems screening for night shift employees. Employer not conducting contact tracing or informing employees of possible Covid-19 exposures. Employer not protecting employees from Covid-19 exposure due to poor face coverings/masks given to patients at the workplace. Employer not providing effective Covid-19 training to employees based on repeated non-compliance from employees about protocols. Employer not separating high risk patients from low risk patients exposing employees to infection. Employer not posting Covid-19 guidelines (sign/posters) at entrances providing instruction on hand hygiene, respiratory hygiene, and cough etiquette (there are in some staff areas, but not in all patient areas). Employer not providing quarantine guidance/enforcement for those with exposure to Covid-19 positive patients. Employer not doing regular/mandated testing/screening of staff members/employees.
