1. The establishment has flies and spiders and it's always very dirty and not cleaned properly; the sinks have mold in them. 2. The general manager has tested positive for COVID after having symptoms and still coming in. The other manager's son got it and management is telling employees that because they have not been in direct contact they don't have to be tested. This even though employees have worked with the manager. 3. The establishment was never closed and was still open serving customers the very next day. 4. Employees who thought they had COVID were still required to come into work until their test came back positive even though they were sick and showing symptoms.Source: Osha.gov | Receipt Date: 2020-09-22
