1. Beacon did not enforce that staff wear appropriate masks to prevent the spread of Covid-19. Both staff and managers would come into the home daily without masks. 2. The employer did not provide enough disinfectant, appropriate masks, and had knowledge that certain staff went out of state but would allow them to work. 3. Two staff members tested positive and this prompted the entire woodland home to be tested. Both employees and residents ended up testing positive. Employees who weren't feeling well were required to stay at work and continue working.
